Job Description:

Mission: Advancing inclusive communities where every person, regardless of circumstances, have the right and opportunity to live their best lifeCore Values: We believe each person should:Be included, respected, and valued.Have ongoing information, experiences, and expectations from which to make choices.Have a variety of relationships.Live and participate in the communities of their choice.Have a career, dreams for the future, and make meaningful contributions.Provides support to individuals with disabilities in developing and/or maintaining skills that will enhance the quality of their lives in the community; assists with personal care, household tasks and daily routines; performs other job related duties as assigned.Essential Functions:Observe and respond to individuals physical, social, behavioral and personal needs;Provide adequate supervision to individuals to facilitate their health and safety;Will:Be fully trained on individuals Individual Support Plan (ISP) and any special plans or therapy.Be fully trained on the individuals Behavior Support Plan (BSP) and report to supervisor as required for incidents.Implement and document on a daily basis the training outcomes and action steps in the ISP as required during the time period the support was delivered.Administer medication to individuals; ensure that administration is timely, in the right dosage and documented on the Medication Administration Record (MAR) as required.Transport individuals to and from appointments, employment and/or community sites;Ensure individuals are appropriately groomed for all activities, employment and appointments,Assist with the planning and implementation of daily activities;Prepare meals or assist with the preparation of meals in accordance with Individual Support Plan and any diet orders, adaptive equipment or mealtime plans;Assist with the daily household chores, to include but not limited to laundry, meal preparation and cleaning;Attend and participate in all relevant agency events, workshops, training and meetings; andPerforms other job related duties as assigned.Secondary Functions:Assist with medical appointments;Performs other job related duties as assigned.
